Instructions for Client Questionnaire

Logging into the Client Questionnaire

Open the web browser on your mobile device or computer (i.e.: Safari, Chrome, Internet Explorer, etc.) and type https://hccscdsurvey.com into the address bar. This will bring up the Log In Screen (see image below).

Enter your SPN and Date of Birth and click .

Once your information has been validated, you will be taken to the first section of the questionnaire.

Completing the Questionnaire

You are required to complete all questions that have a red * at the end of the question:

Once you have answered all required questions in a section, click at the bottom of the page to move to the next section.

In the top right corner of each page, there is an option to . Click this to save your current progress from the previous sections and return to the questionnaire to complete at a later time.

Any time prior to submitting your questionnaire, you may return to a previous section to review or revise your answers.

Once all five sections have been completed, click to send your completed questionnaire to Harris County CSCD.
